black green stools

my baby is 5 months old and having too much cough and cold all d time. he is not pooing regularly now and poos once in 3-4 days.his poos color is combination of green black and yellow. it is also tight and he has to apply lot of pressure while passing stool.and at the same time suddenly watery...i m vey much concerned.pls help. he is on breast feed.
