A few questions...
Did baby latch on properly when you breastfed? Have you had a lactation consultant help you?
What made you think that you didn't have enough milk?
Have you thought of using an SNS (supplementary nursing system) It's a tube connected to a bottle, specially made for a mom who wants to breastfeed her baby if she has a low milk supply.
This SNS will allow you to give your baby the formula while your own milk supply is being built. The more you breastfeed the more milk you will start producing.
The SNS is great because you can still bond with your baby even if he/she is getting most of their nourishment from the formula.
Since your baby is already accustomed to a bottle, it might be a little difficult getting him/her to drink from the tube and on the breast, but you won't know unless you try.

You will also need to pump in-between feedings to increase breast milk even further.
Try not to stress, this can cause you to produce even less milk.

Don't get discouraged. It can take a while for your milk to come in, and some moms never develop a full milk supply. Take pride in the fact that you are providing your baby with a unique bonding experience... even if you cannot produce all of baby's food.
